Essential Functions:
1) Coordinate program services, utilizing applicable standards
2) Maintain client records according to standards
3) Enter appropriate information into funding agency's enrollment system
4) Conduct annual assessments (including NC Snap) and develop service plans
5) Take attendance daily
6) Counsel clients regarding problems: serve as liaison with families/personal representatives
7) Advocate on behalf of clients, families and/or personal representatives
8) Serve on assigned committees
9) Attend applicable meetings/trainings to support programs
10) Coordinate transportation for assigned clients, including billings/payments.
11) Serve on Case Records Review Committee, Admissions/Discharge Committee
12) Serve as QP for ADVP/CLFS
13) Conduct quarterly "Big Rap" client meetings
14) Submit weekly program billings to bookkeeping by Tuesday
General Duties
1) Meets work deadlines and performance/productivity quotas/requirements on schedule
2) Works cooperatively with staff, clients and personnel from other agencies/companies
3) Sets a good personal example and serves as a positive role model for clients
4) Follows all agency administrative and personnel policies/procedures and work rules
5) Assists/performs other duties or special projects/assignments as requested
6) Maintain clean and safe work environment
7) Follow all quality standards/procedures
Requirements
Bachelors Degree/equivalent in human service area preferred: 4 years required in developmental disability field: 1-2 years supervisory experience This position requires NC Health Registry Check
